Authorization for Medication Form
If your child needs to have medication administered during the school day, please complete the Authorization for Medication Form [hyperlink the name of the form to the form.] Please note that your child’s health care provider must sign the form in order for it to be valid. A completed form must be on file prior medication being administered to your child at school. This requirement includes Over the Counter medications as well. One exception is that 7th and 8th grade students may keep OTC medication in their backpacks but may not share it with others.
School Physical Physical Qualifying Form
Students entering 7th grade, must have a Sports Qualifying Physical form on file prior to participating in any extracurricular athletic events. This physical is valid for three years.
Animals in the Classroom Form
If you wish to bring an animal into the classroom for “show and tell” purposes, a request must be filled out and turned in to the school principal ahead of time. Please download and fill out the form below to make a request to have a pet brought into the classroom.
